You MUST take the following steps to ensure that your referral can be processed as quickly as possible:
- Before submitting any information to Sparta Health, it is important that the referred individual understands why they are being referred. You must discuss the referral with them, and they must agree to this.
- Please ensure the form below is completed fully to assist Sparta Health in providing the referred individual with appropriate counselling support services.
Sparta Health will then:
- Request an electronic written consent form from the referred individual. Please ask them to check their email (and junk/spam mail).
- Once the completed and signed consent form is returned, Sparta Health will make arrangements to place the referred individual with an appropriately qualified and experienced counsellor. You will be kept updated throughout the process.
