Information We Collect About You

Service Provision

In order to provide our health, wellbeing, absence management & rehabilitation services to you we may collect the following personal information about you:

  • Name, postal address, contact number and email address, name of employer, postal address and contact details of employer, areas of expertise/practice, details of relevant industry or professional accreditations, qualifications and past experience.
  • General Practitioner contact details, medical information relevant to your injury / condition, pre-incident medical information relevant to your injury/condition; treatment details relevant to your injury / condition; pre-incident treatment details relevant to your injury / condition.

This information is collected and processed while under contract.

Website & Marketing

Through the use of our website, the following information may be collected:

  • When registering for our newsletter, your name and email address.
  • Any personal data entered into our referral form.
  • Comments / messages sent via the website (email link or form).
  • We use Google Analytics to collect information related to usage of website. Google Analytics stores details on pages visited, length of time spent on pages and what was clicked. This information does not include personally identifiable information and cannot be used to identify you.

Basis for Processing

Service Provision

We will process personal data, based on an existing or pending contractual agreement.

Website & Marketing

We will only process data for marketing purposes on the basis of your consent.

How Your Information Will Be Used

Service Provision

Your information will be added to our systems and will be used in the provision of our service to you. It will be used in the following ways:

  • To communicate and correspond with you, your representative (where applicable) and the funding party throughout the time we are engaged as your health, wellbeing, absence management & rehabilitation service provider.
  • To make decisions regarding appropriate treatment pathways and make recommendations for health, wellbeing, absence management & rehabilitation programmes and support.
  • To communicate with any medical professionals that are involved in your health, wellbeing, absence management or rehabilitation programme prior to and during our involvement.
  • To communicate and correspond with an employer (where relevant), or another instructing party, to assist in the arrangement of return to work or adjustments within the workplace.

Website & Marketing

Your information will be retained and added to our systems and will be used in the provision of our service to you. It will be used in the following ways:

  • Provide you with our newsletter.
  • Help us respond to service enquiries.
  • Google Analytics aids with future improvements to our website, but assisting us in understanding the general usage of our website.

Who We May Share Your Information With

As per the above, your information, as well as copies or your medical / treatment information relating to your injuries / condition may be shared with relevant personnel within Sparta Health whom require access for legitimate business purposes, as well as with your employer (for the purposes of providing our services and to assist with your return to work), your representatives (where applicable) and the funding party.

Cookies

Cookies are small files that a site or its service provider transfers to your computer’s hard drive through your Web browser (if you allow) that enables the site’s or service provider’s systems to recognise your browser and capture and remember certain information. They are also used to help us understand your preferences based on previous or current site activity, which enables us to provide you with improved services. How Long Will We Keep Your Data?

Service Provision

We will not retain your data for longer than necessary for the purposes set out in this Policy. Where we have provided rehabilitation services we retain your data for a minimum of 7 years. These records are classed as medical records and Sparta Health comply with the Records Management: NHS Code of Practice which specifies the length of time health records should be kept, dependent on the type of record held.

Website & Marketing

You may unsubscribe at any time using the unsubscribe link on the newsletter email or by contacting us directly. We will retain your personal information for as long as you wish to remain on our newsletter mailing list.

Lawful Basis for Processing

We will only process your information where it is necessary to support the legitimate interests of our business or those with whom we may have shared your information with except where such interests are overridden by your interests or fundamental rights and freedoms which require the protection of personal data.

International Transfers

Sparta Health will not transfer your data to countries outside the European Economic Area.

Your Rights

  • Know what specific personal data we hold on you in our records, and request a copy of that data (the right of access).
  • Ask us (in certain circumstances) to supply you with some of the personal data we hold about you in a structured machine-readable format and/or to provide a copy of the data in such a format to another organisation (the right to data portability).
  • Have your personal data deleted from our systems (in certain circumstances) where we no longer have any legal reason to retain it (the right of erasure or to be forgotten).

  • Restrict the processing of your data (in certain circumstances); which means that we would need to secure and retain the data for your benefit but not otherwise use it (the right to restrict processing).

  • Withdraw consent to having your personal data used for marketing purposes / holding your personal data if we have no legitimate reasons to do so (the right to object).
  • Have your personal data transferred to another company (the right to data portability).

If you choose to restrict the processing of your data, we may not be able to continue to provide you with our services.

We will comply with any restriction on the processing of your data unless fulfilling your request would reveal information about another person, or if you ask us to delete information which we are permitted or required by law to keep.
